Physical Therapy Services at Your Home!

 

Our program is not for designed for everyone in the community. We specialize in local resident members, including senior citizens, who are able to get out into the community.  This group may have difficulty, or find it inconvenience or uncomfortable to get out into the community.

They would prefer to have therapy in their own home.

 

Rehabilitation services are provided by a qualified, licensed physical therapist and coordinated with the patient's physician.

  • Have you received a physical therapy prescription/referral from a recent visit to your doctor?
  • Do you have limitations performing daily activities but you do not qualify for home health nursing or physical therapy services?
  • Do you find it difficult or inconvenient to attend clinic-based physical therapy?

We will coordinate with your doctor regarding your prescription/referral. Then we gather some basic information about you and set up an appointment time.

At your appointment we identify your difficulties and determine what you would like to accomplish with physical therapy.

We provide an evaluation, treatment and start working toward attaining your goals on your first visit.

We currently do not accept insurance plans. We operate on a cash basis. Clients pay be check at the end of treatment.

Patients may qualify for treatment with little or no out-of-pocket costs under certain circumstances!

We are not a skilled home health agency and do not provide skilled nursing or rehabilitation services to homebound patients.

  • The right to take part in  decisions about medical treatments that work toward your functional goals.
  • The right to  refuse treatments that you are not comfortable with.

 

   Patient's Responsibilities

As a patient involved in our services, you are responsible for:

  • Providing accurate and complete health information.
  • Participating in planning your functional goals and treatments.
  • Asking questions you do not understand during the evaluation and treatment.
  • To consider the rights of your therapist, physicians and program staff.
  • Complying with the treatment plan and goals you have agreed to.
  • Paying for the services rendered at the end of each treatment.
